The staff will administer medicine upon written authorization only by the parent.  Written authorization may be made using the standard Giggles Learning Center medication form.  In each case the parent should complete, sign, and date the form and deliver it, with the labeled medicine, to the Director.

STATE LAW REQUIRED THAT ALL MEDICINES MUST BE:

In the original container;

clearly labeled with the name of the medicine;

clearly labeled with your child's name;

if a prescription, labeled with a prescription number;

clearly labeled with the dosage to be administered; and

clearly labeled with the date.

The school will notify you in case of any adverse reaction to the medication.
